Notice of dissolution

NOTICE OF DISSOLUTION 
 OF PARK PLACE 
SENIOR LIVING, LLC

You are hereby notified pursuant to Minn. Stat. 322C.0704 that Park Place Senior Living, LLC, a Minnesota limited liability company (the “Company”), has dissolved. Claims against the Company must be in writing and must be mailed to: Benedictine, Attn: Legal Counsel, 6499 University Ave. NE, Suite 300, Minneapolis, MN 55432. Claims must include the claimant’s name, address, phone number, and email address and must specify the amount claimed and details of the claim. A claim against the Company is barred unless an action to enforce the claim is commenced within five years of the date of this notice.
